What is Home Health Care?
Home Health Care is designed to help you recover from an illness or surgery at home, providing the supports you need to manage a chronic or progressive illness, take your medicine, care for a wound, walk or climb stairs, learn how to bathe or dress on your own again, or achieve other health milestones so that you can live independently at home and avoid going back to the hospital.
When a physician, nurse, hospital discharge planner or case manager suggests professional, skilled services at home to assist with you or a loved one with health care needs, the VNA is your partner to give you the most compassionate care right at home.Home Health takes a holistic approach to your health care and can include a broad range of services such as:
A Home Health nurse is available by phone 24 hours a day, 7 days a week.
What if my condition worsens?
If your illness progresses, the VNA will provide an enhanced level of nursing support to help you manage your symptoms. This is called Palliative Care, and focuses on providing you with relief from the symptoms, pain, physical stress, and mental stress of a serious illness—whatever the diagnosis.
Hospice services are available for those facing their end of life journey and focus on physical, emotional and spiritual needs.

Are these services covered by insurance?
Services are generally covered by Medicare, Medicaid or other third party insurance provider for those who are limited to home due to illness or injury. Free or reduced-fee care is available to those who qualify.
The VNA's home health services are fully licensed by the Commonwealth of PA, certified by Medicare and Medicaid. For your convenience, the VNA staff submits all insurance claims on your behalf.
