Case Starter (Nurse Aide only)

Occupational Therapist Part-time
The Visiting Nurse Association of Indiana County, Pennsylvania, is seeking a qualified Occupational Therapist to join our home health team on a part-time basis.
The licensed occupational therapist will:
Make 10 to 15 home visits per week to patients in the Indiana County, PA. area
Be responsible for the initial occupational therapy evaluation and assessment of functional status, endurance, visual coordination, motor skills, environmental hazards or barriers to independent living.
Set up and implement the treatment plan, and participate with nursing in multidisciplinary care.
Report and record for home health and hospice patients, under the direction of the patients physician
Identify equipment needs and environmental adaptations
Educate patient, caregivers and family in rehabilitative care
Supervise the Certified Occupational Therapist Assistant in implementing the plan of care
Revise therapy plans as needed to enhance outcome for patients
Responsible to the Therapy/Ancillary Services Supervisor
Requirements: Valid PA drivers license and a reliable vehicle- Paid mileage and travel time. Criminal History and drug screening.
